belief-source-metadata-is-file-level
IN premise
Belief source tracking uses flat file-level pointers (source path + source_hash) with no structural awareness of where within a document the claim originated — section, page, or paragraph level provenance is not supported.
Summary
The system can only track which file a claim came from, not where inside that file it was found. This means if a source document changes, you can't tell whether the specific section backing a claim was affected — you have to treat the entire file as potentially invalidated, which makes staleness detection coarser than it could be.
